The information collected on this application form is solely intended to determine applicant suitability for a volunteer position. Any information collected on this application form will remain strictly confidential and any personal information disclosed for the purpose of obtaining a reference will be treated as confidential and private.
By submitting volunteer form below, I consent and authorize Sudbury Cedar Place to obtain and verify the information outlined in the Volunteer Application Form and release Sudbury Cedar Place from any claim whatsoever relating to information during their verification process. I authorize any person I have listed as a reference to disclose any information regarding my suitability for a volunteer position. The information provided on this application form will be treated in accordance with the Personal Information and Protection and Electronic Documents Acts (PIPEDA). It will be used by Sudbury Cedar Place and its partnering agencies for the purpose of selecting, placing and communicating with volunteers. The information will not be shared with any outside organization. Completing this application constitutes consent to the collections, use or disclosure of this information for the purposes described.
This is an application to volunteer with Sudbury Cedar Place for which there is no monetary compensation. I understand that if I am accepted as a volunteer with Sudbury Cedar Place, I am agreeing to: Fulfill the orientation and training requirements for the volunteer role; A minimum commitment of 6-12 months & 2-4 hours per week; Abide by the policies and procedures of Sudbury Cedar Place.
CONFIDENTIAL AGREEMENT
By submitting the form below, I acknowledge that in the course of my volunteer work with Sudbury Cedar Place, confidential and privileged information maybe shared with me in writing, verbally, electronically or in any other medium. The information is given in confidence and trust. I agree that during the course of volunteering with Sudbury Cedar Place, and after my departure, I will keep all such information strictly confidential and not disclose such information to anyone under any circumstances. I understand that a violation of these obligations may be a cause for termination of my services with Sudbury Cedar Place without notice.
LIABILITY RELEASE
By submitting the form below, I acknowledge that Sudbury Cedar Place cannot be held responsible for the loss, damage or theft of any belongings brought to any Sudbury Cedar Place program. Sudbury Cedar Place cannot be held responsible for any and all claims for damages arising as a result of any accident or injury or otherwise sustained by participants arising from Sudbury Cedar Place activity or program. I also agree to sign the Volunteer Waiver & Release provided to me along with this application and to promptly return a signed copy to Sudbury Cedar Place.
PERMISSSION TO USE PHOTOGRAPHY
By submitting the form below, I grant to Sudbury Cedar Place, its representatives and employees the right to take photographs of me and my property in connection with my volunteer role. I authorize Sudbury Cedar Place, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that Sudbury Cedar Place may use such photographs of me with or without my name and for any lawful purpose including for example such purposes as publicity, illustration, advertising and Web content.
VOLUNTEER DECLARATION
Sudbury Cedar Place’s clients are considered a vulnerable group as defined by the RCMP guidelines.
Therefore, we are required under the Criminal Records Act - R.S.C., 1985, c. C-47 (Section 6.3), to ensure we screen all volunteers. As a volunteer, we request that you please complete the form prior to volunteering with us.
I understand that I may be asked to complete a Vulnerable Sector Check depending on my volunteer with Sudbury Cedar Place. By submitting the form below, I declare that: I have no convictions under the Criminal Code of Canada, up to and including the date of this declaration, for which a pardon has not been issued or granted under the Criminal Records Act.
